June 23 – June 27, 2008

Performances will be on Saturday, June 28th at 5:30pm
and Sunday, June 29th at 8:30 & 10:00am.

COST
$30 per child
$20 per child (if parent volunteers in program)
$75 maximum (for families with more than 2 children in program)

Permission to Use Photos

We take every opportunity to capture Mountain View's precious children in photos. Your approval is required to use your child(ren)'s photos, as well as any photos that may be taken of you, for Mountain View publications, including, but not limited to, our website, newsletter, brochures, and other community publications.

If you have more than 2 children to register, you will need to submit another registration form.

You may bring your payment to the church office during regular business hours (M-F, 8:30am-3:30pm) or to the church during worship hours. Please make checks payable to Mountain View Lutheran Church, and write your child(ren)'s names on the memo line.
